Job Summary
Coordinate client onboarding, service agreements, and budgets while maintaining accurate records to ensure aged care funding and compliance. Assist with care plans, risk assessments, and documentation including incidents and progress notes within the CRM. Provide administrative support to Registered Nurses and coordinate communication between clients, families, and Support Workers. Monitor client information and service documentation to support continuous improvement in dementia-focused service delivery. Required Qualifications
- Experience in aged care, community services, health or a similar environment
- Experience coordinating services and supporting clients, particularly people living with dementia
- Strong administrative, organisational and time management skills
- Excellent communication and interpersonal skills
- Strong attention to detail and the ability to manage competing priorities
- An understanding of aged care funding, compliance and service delivery requirements
- Experience using CRM systems and Microsoft Office
- A current National Police Clearance or willingness to obtain one
- Knowledge or experience with the Support at Home Program and CHSP
- Experience with My Aged Care and aged care funding requirements
