Job Summary
HOST Intensive Case Manager collaborates with clinical, medical, housing, peer support, and shelter leadership to provide comprehensive care, crisis response, and low-barrier clinical services to clients enrolled in HOST, many services delivered in Snohomish County including streets, shelters, jail, and service centers. Responsible for referral, outreach, intensive case management, and coordination to transition clients to ongoing care for a caseload of 25 or fewer individuals with mental health disorders. This role emphasizes advocacy, linkage to community resources, and collaboration with external providers, with a focus on SUD and co-occurring disorders, serving populations that are homeless or housing-insecure. Essential skills include de-escalation, motivational interviewing, advocacy within the criminal justice system, and ability to work with individuals with active substance use and untreated mental illness.
Required Qualifications
- Licensed by the State of Washington as Substance Use Disorder Professional (SUDP) or SUDP Trainee (SUDPT)
- 2 years of experience working with adults who are experiencing or who have experienced homelessness, mental illness and/or substance abuse disorders
- street outreach experience and/or Peer Counselor credential a plus
- Knowledge of de-escalation skills, motivational Interviewing and patience with persons who have not succeeded in traditional service settings
- Must have a current WA State driver's license and an insurable driving record
- FT - 40 hours per week. Flexible hours.
