HOST Intensive Case Manager

Job Summary
Conduct intensive case management, outreach, and engagement for a caseload of 20 or fewer adults experiencing homelessness, severe mental illness, and co-occurring disorders. Perform assessments, develop treatment plans, and facilitate linkages to collaborative resources while advocating for client access to community services and rights. Provide crisis response and low-barrier clinical support across Everett and Snohomish County, including street outreach, shelters, and jail settings, collaborating with psychiatric providers, nursing, and peer specialists to ensure successful transitions to ongoing care. Work Monday through Friday day shifts, coordinating with external agencies and ERC programs to address acute needs and increase stability.
Required Qualifications
- 2 years of experience working with adults who are experiencing or who have experienced homelessness, mental illness and/or substance abuse disorders
- Knowledge of de-escalation skills
- Knowledge of motivational interviewing
- Patience with persons who have not succeeded in traditional service settings
- Street outreach experience
- Peer Counselor credential
