Job Summary
Host Home Providers offer in-home placement to adults with disabilities, delivering personal and social support in a home/community setting. Responsibilities include providing personal assistance, ADL support, transportation to activities/appointments, documenting services, administering medications as needed, and implementing activity plans with the individual in care. The role includes crisis intervention and trauma-informed care, 24/7 on-call support, and collaboration with a multidisciplinary clinical team. Training is provided, and compensation is based on the level of care required, with supplemental room and board possible up to $797/month. Requirements include age 21+, GED or diploma, reliable transportation, valid driver’s license, ability to pass a background check, and an appropriate living environment. Preferred qualifications include CNA, LPN, or RN certification and knowledge of developmental disabilities, mental health, and trauma.
