Job Summary
Implement Catholic Health's Patient Experience Program on specific hospital campuses to ensure patients and families receive comfortable, compassionate, and safe care. Serve as the key point of contact between unit leadership and patients, actively listening to concerns, escalating issues, and performing service recovery. Round on patients and families regularly to develop trust, facilitate communication with the care team, and ensure real-time follow-up on feedback. Track and trend patient data, complete reports, and conduct post-discharge follow-up calls to collect additional insights. Provide one-on-one coaching for leaders, educate staff on patient experience improvement using I-CARE values, and collaborate on process improvement projects. Assist the PX Director with in-depth analysis of all patient metrics and qualitative data.

- 1–2 years of work experience participating in performance improvement programs
