Job Summary
Conduct post-acute hospitalist work for Skilled Nursing Facility patients transferred from acute rehab, including admitting, discharging, and daily patient visits. Manage an average of 15 patients daily with support from PM&R physicians and an experienced APP team. Work full-time Monday through Friday from 8am to 5pm, with early departure permitted once duties are complete but phone availability required until shift end. No night shifts required; share night coverage with other Hospitalists from home.
Required Qualifications
- Hospitalist experience
- Post-acute Hospitalist experience
- Skilled Nursing Facility (SNF) experience
- Admitting patients
- Discharging patients
- Seeing patients on a daily basis
- Treating patients transferred for acute rehab services after hospital stay
- Available by phone until shift ends at 5pm
- Ability to see an average of 15 patients per day
