Hospitalist Care Coordinator

Manage incoming calls from pharmacies, patients, and families regarding concerns voiced by patient care staff. Serve as the liaison between home health, outcome managers, documentation specialists, and the physician Hospitalist team. Complete patient documents including FMLA and insurance forms related to medications while evaluating clinical records. Utilize Epic for computer work and maintain strong organizational and record-keeping skills. Requires current NC RN/LPN license, five years of experience, and knowledge of MRH processes.
