Hospital Social Worker

Conduct cross continuum care coordination, psychosocial assessments, and interventions across clinic, provider practice, urgent care, phone care agency, skilled care, and other sites. Provide customer service to patients, families, physicians, and care teams regarding discharge milestones and progression to the next level of care. Model and orchestrate consistent, professional communications while maintaining proactive patterns with colleagues and staff. Delegate responsibilities appropriately while remaining accountable for outcomes to ensure medically appropriate transitions of care. Perform these duties as a PRN position with a variable shift schedule.
