Hospital Nurse Liaison (PRN)

Establish Hospice of the Piedmont presence in hospitals by attending rounds, care conferences, in-service meetings, committee meetings, and new staff orientations. Coordinate discharge plans with hospital case managers, patients, families, and hospice staff for newly referred patients, providing consultation and follow-up. Inform hospital patients and their families about Hospice of the Piedmont services while educating medical professionals on pain management, hospice care, and reimbursements. Promptly transfer patient information from hospital systems to the EMR and advocate for current hospice patients admitted to the hospital or ER. Maintain confidentiality and demonstrate familiarity with agency policies, state, and federal hospice regulations. Contribute to marketing and business development plans to ensure census growth. Participate in research, quality improvement, or other end-of-life care studies. Work 8:00 a.m. to 5:00 p.m. on a PRN basis with flexibility for weekday and weekend shifts. Requires a Virginia RN license with two years of terminal care experience.
