Job Summary
Nurse III, Hospital Liaison Nurse (HLN) performs highly complex nursing work coordinating and organizing nursing services for SSLC individuals in hospitals or healthcare facilities. Responsibilities include conducting hospital rounds, performing outreach and liaison activities with local hospitals, observing and communicating patient status, providing consultation, training and technical assistance to DSPs and staff, ensuring discharge planning and information exchange, developing and explaining hospital-related policies/procedures, maintaining comprehensive documentation (Hospital Liaison Reports, IRIS entries, discharge plans), coordinating with interdisciplinary teams for discharge back to the SSLC, and maintaining compliance with regulations. The role requires strong nursing skills, knowledge of healthcare laws and regulations, ability to train staff, effective communication, and the ability to work independently with various teams. The position involves on-site hospital liaison duties, cross-training, and contributing to continuous improvement in hospital liaison processes; CPR certification is required and ongoing professional competencies are expected.
