Job Summary
Hospital Liaison responsible for coordinating the referral process to support facility census and quality mix goals in an assigned market. Performs clinical and financial patient needs assessments, builds relationships with hospitals, physicians, case managers, and referral sources, handles benefits verification and pre-certification with Admissions Directors, provides admission recommendations based on clinical and financial evaluations, ensures timely patient assessments and orderly transitions into long-term care facilities, maintains CRM/database records, monitors bed availability and waiting lists, stays informed on Medicare/Medicaid/Managed Care regulations, supports marketing initiatives, and participates in process-improvement initiatives to enhance workflow and customer experience.
Required Qualifications
- RN license preferred
- LPN or other healthcare backgrounds considered
- Minimum 3+ years experience in long-term care or related healthcare field
- 1–2 years of sales, marketing, or case management experience preferred
- Strong understanding of referral processes and long-term care operations
- Excellent communication and relationship-building skills
- Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
- Strong organizational skills with the ability to manage multiple priorities
- Ability to work independently and make informed decisions in a fast-paced environment
