Job Summary
Develop and maintain relationships with hospital case managers, discharge planners, physicians, and healthcare professionals to facilitate patient admissions and ensure seamless transitions from acute care to skilled nursing facilities. Conduct regular hospital visits to identify referral opportunities, review patient referrals, and collaborate with clinical teams to determine appropriate placement. Educate referral sources about services and clinical programs while tracking activity and providing performance reports. Meet census growth and referral development goals through networking events and community outreach initiatives.
Required Qualifications
- Minimum of 2 years of experience in healthcare sales, business development, admissions, case management, or hospital liaison roles
- Valid driver's license and reliable transportation
Desired Qualifications
- Bachelor's degree in Healthcare Administration, Marketing, Business, Nursing, Social Work, or related field preferred
- Experience working with skilled nursing, rehabilitation, long-term care, home health, hospice, or hospital systems preferred
- Strong understanding of hospital discharge planning and post-acute care services
- Excellent communication, relationship-building, and presentation skills
- Ability to work independently and manage a territory effectively
- Strong organizational and time-management abilities
