Job Summary
Hospital Liaison/Case Manager to provide liaison services with state psychiatric hospitals, ensure continuity of care for individuals transitioning from long-term hospitalization to community-based services, and deliver case management for adults with mental illness, SUD, developmental disabilities, or co-occurring disorders across forensic and civil unit populations, including NGRI and civil commitments; requires QMHP, Restoration Counselor training, Bachelor's in a Human Services field, valid VA driver's license, travel to state hospitals, and flexible scheduling.
Required Qualifications
- Bachelor's degree in a Human Services field
- QMHP certification
- Must complete all required Restoration Counselor training
- Knowledge and experience in assessment, counseling, case management, and treatment planning
- Valid Virginia driver's license
- Ability to travel to state hospitals and throughout the agency's catchment area
- Flexible schedules may be required to meet program and agency needs
