Job Summary
Conduct screenings and assessments with new mothers and babies to determine eligibility for Healthy Beginnings services. Collaborate with hospital staff and Birth Registrars to ensure accurate information, while identifying client needs, risk factors, and protective factors. Connect eligible families with program services and community resources, serving as a liaison between hospitals, families, supervisors, and community partners. Maintain accurate documentation and program data while meeting established outcomes, attending trainings, and participating in staff meetings and supervision. This role requires scheduling flexibility to accommodate hospital shifts, including occasional evening and weekend hours.
Required Qualifications
- Bachelor's degree in Human Services (preferably in a field such as social work, developmental psychology, and/or early childhood education)
- Bachelor's degree in a different field with a minimum of 2 years' experience with children and families
- Minimum of two (2) years' experience in child welfare, primary prevention services, child development or related field
- Schedule includes occasional evening and weekend hours as needed
- Must be 21 years or older
- Bilingual English/Spanish required
- Valid Florida's driver's license is required
- Successful completion of a background check through Clearinghouse
