Job Summary
Hospital Discharge Coordinator responsible for intake of Case Manager inquiries and documenting order information to meet coverage criteria; coordinate in-hospital set-ups for home-going oxygen and modalities; liaise between MSC and facility staff; maintain open communication with patients/clients and referral sources; market services to Case Managers and referral sources within facilities; document communications and keep frequency with referrals; perform other duties as assigned.
Required Qualifications
- Education: Graduate of an accredited high school or GED equivalent
- One year (minimum) of progressively responsible medical customer service experience
- Excellent skill sets in communication and customer relations
- Detail oriented with excellent interpersonal and organizational skills
- Must be able to perform extensive driving as required for the position
- Must possess a valid driver’s license
- Must be able to lift and carry up to 50 pounds
- Must be able to walk, stand, bend, stoop, and perform setup tasks throughout the workday
- Must be able to safely operate equipment and follow all safety protocols
