Job Summary
Build and maintain relationships with hospitals, case managers, physicians, and healthcare providers across Central Vermont and Grafton County, New Hampshire. Coordinate referrals and facilitate smooth transitions of care while representing our skilled nursing and rehabilitation centers with professionalism. Work closely with facility leadership to support census growth and community outreach. This full-time position requires travel throughout the assigned territory and a valid driver's license.
Required Qualifications
- Travel throughout the assigned territory is required
- Valid driver's license and reliable transportation required
- Self-motivated with the ability to manage a travel-based territory
Desired Qualifications
- Experience in healthcare marketing, admissions, case management, nursing, or hospital liaison services preferred
- Excellent communication and relationship-building skills
