Job Summary
Hospital Based Case Coordinator (HBCC) – Winnipeg: Provides referrals intake, assessments for Home Care program eligibility, coordinates and evaluates care plans to support discharge from acute care, assists in determining suitable care environments, liaises with healthcare teams, agencies, and families. Requires RN qualification, registration, and strong assessment, care coordination, and communication skills; experience with interRAI-HC and Procura software is preferred; shift coverage includes days, evenings, weekends; in-person work arrangement; drive or access to vehicle may be required.
Required Qualifications
- Graduate of an approved Registered Nursing education program
- Registration with the College of Registered Nurses of Manitoba (CRNM) required
- CPR training as per policy
- Driver's license and use of a vehicle
