Job Summary
Visit Registered Nurse responsible for assessing and identifying patient/family needs, planning, implementing, evaluating and documenting individualized hospice nursing care per the interdisciplinary plan of care. Provides care coordination and collaborates with the Interdisciplinary Group, supervising LPNs and CNAs. Assigned patients may be in home, skilled nursing facility, assisted living facility, or acute care; may include home visits, unplanned visits, and admissions coordination. Schedule: Monday–Friday, 8:30 AM–5:00 PM with some on-call rotation. Licensure in Kentucky and/or Indiana required; BSN desired; valid driver’s license required; EMR experience preferred.
Required Qualifications
- RN licensure in Kentucky and/or Indiana as required by designated position
- BSN desired
- Certification in hospice/palliative care preferred
- One year of nursing experience preferred
- Knowledge of palliative care helpful
- Must have a valid driver’s license
- Use of electronic medical record strongly preferred
