Hospice Transition Nurse (RN)

Manage GIP patients and act as a resource for hospitals, physicians, and nursing homes by navigating EMR systems to support hospice admissions. Conduct initial goals of care meetings with patients and families to inform them about St. Croix Hospice programs and services. Collaborate with business development to sustain and increase referral sources while documenting all visits and phone calls. Assist with patient transitions, coordinating comfort medications, DME needs, and admission consents with the Medical Director. Mentors new RNs, attends interdisciplinary care meetings, and establishes relationships with hospital case managers, social workers, and physicians. Requires current RN license, valid driver's license, and 1+ years of hospice experience.
