Job Summary
Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, and health groups to apprise them of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative responsible for education related to provided services. This role requires a graduate of an approved school of clinical education, state licensure, and at least one year of home care experience, including knowledge of federal, state, and local hospice regulations. Candidates must possess a valid driver's license, maintain automobile liability insurance, and have dependable transportation to safely drive in all weather conditions.
Required Qualifications
- Must be a graduate of an approved school of clinical education
- Must be licensed in the state where they currently practice
- Must have at least one year experience in home care
- Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
Desired Qualifications
- Demonstrated experience with and knowledge of hospice is preferred
