Job Summary
Hospice Transition Care Nurse coordinates inpatient admissions and oversees the transition of patients from hospital to home hospice. Works with physicians and mid-levels to determine eligibility for hospice care, conducts daily patient visits in the hospital, educates patients about home hospice care, implements patient care plans, and participates in biweekly meetings. Must hold Georgia RN licensure; 2 years of patient care experience required; 1 year hospice/palliative care experience preferred. EMR experience with KanTime preferred. Monday–Friday, 8:30am–5:00pm. Benefits include salary in the $85k–$92k range, health/dental/vision insurance, mileage reimbursement, PTO, holidays, and 401(k) with match.
