Job Summary
Hospice Team Coordinator position overseeing delivery of hospice care, coordinating patient records and admissions, using HCHB/EMR systems, ensuring billing alignment with level of care, and serving as central point of contact for patients, families, and the interdisciplinary team. Requires managing referral intake, admission documentation, CTI and orders to admit, coordinating IDT activities, and strong organization, communication, and data-entry skills. High school diploma or GED required; 2+ years in medical records or office support preferred. Emphasis on patient-centered care, compliance, and collaboration within Compassus’ home-based care model.
Required Qualifications
- High school diploma or GED
- Experience with medical records or medical office preferred (2+ years)
- Ability to communicate effectively in English
- Strong written and verbal communication skills
- Attention to confidentiality and HIPAA/privacy
