Job Summary

Complete initial psychosocial assessments and establish individualized plans of care for patients and families. Provide emotional support, education, crisis counseling, and advocacy while communicating regularly with caregivers and facility partners. Assess bereavement risk, support grief services, coordinate community resources, and assist with funeral planning. Participate in interdisciplinary team meetings, plan-of-care reviews, and volunteer coordination for Veteran pinning ceremonies. Maintain accurate documentation, adhere to the Social Work Code of Ethics, and adjust to frequently changing demands. Work part-time (24 hours per week) with a 3-day schedule including weekend rotation and holiday coverage.

Required Qualifications

  • Bachelor's in Social Work (BSW) from an institution accredited by the Council on Social Work Education
  • Bachelor's degree in psychology, sociology, or other fields related to social work
  • one year in a healthcare setting
  • Certification or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin
  • Valid Wisconsin Driver's License
  • reliable transportation
  • proof of current automobile insurance coverage
