Hospice Social Worker

Hospice Social Worker collaborates with an interdisciplinary patient-care team to provide compassionate psychosocial support to patients and families, conducts psychosocial assessments, delivers counseling and crisis intervention, coordinates care planning and referrals to community resources, and supports bereavement activities. The role requires a Bachelor’s degree in social work, current licensure in the practice state, and adherence to HIPAA; licensure levels include LBSW, LMSW, LICSW, or LAMSW. The position operates Monday–Friday, 8:00 am–5:00 pm, as a full-time role based in Nixa, MO, with responsibilities including documentation, care coordination, and collaboration with the clinical team. Must have reliable transportation, valid driver’s license, and proof of auto insurance, and be able to pass background, consumer report, and drug testing.
