Job Summary
Social Worker for hospice care provides psychosocial assessments and counseling to patients and families in terminal illness contexts, coordinates with the interdisciplinary care team, and connects families with community resources. Responsibilities include assessing psychosocial status, conducting social evaluations, crisis intervention, ongoing bereavement support, assisting with advanced directives, maintaining HIPAA-compliant records, and educating families. Work occurs in patients’ residences/home settings with collaboration across care teams and community agencies; requires licensing and travel to patient homes, and adherence to care plans and CHAP/regulatory requirements.
Required Qualifications
- Bachelor’s degree in social work from a CSWE-accredited program
- Current licensed in the state(s) of practice
- Licensure levels: LBSW, LMSW, or LICSW
- Ability to pass background, consumer report, and drug test
- Valid State Driver’s License and automobile in good working order
