Job Summary
Provide initial psychosocial assessments, plan of care, ongoing psychosocial counseling, bereavement services, community education, outreach, and referrals to Hospice patients and families. Maintain a Master's Degree in Social Work, Licensed Independent Social Worker licensure, and a minimum of three years of clinical experience, preferably in healthcare. This role supports the hospice team by delivering direct psychosocial support and resource navigation for patients and their families.
