Job Summary
Hospice Social Worker providing emotional support, counseling, and practical guidance to patients and families navigating end-of-life care; assesses psychosocial needs, collaborates with the interdisciplinary team to develop care plans, assists with advance directives and discharge planning, connects patients to community resources, documents assessments and interventions, and travels locally to patient homes and facilities to deliver compassionate, dignified care.
Required Qualifications
- Master’s degree in Social Work (MSW) from a CSWE-accredited program
- Licensed in Texas as an LMSW or LBSW, in good standing with the Texas State Board of Social Worker Examiners
- Experience in hospice or palliative care (1+ year preferred)
- Knowledge of local community resources and ability to connect patients and families with support
- Strong interpersonal and communication skills
- Valid Texas driver’s license, reliable transportation, and current auto liability insurance required for travel throughout the service area
