Job Summary
Explain hospice services and Medicare benefits to patients and families, obtaining informed consent documents as requested. Complete and submit appropriate documentation and paperwork to facilities at the completion of patient visits. Participate as a member of the interdisciplinary team, including development and implementation of the plan of care. Provide psycho-social support and counseling services to the patient and family, supporting the involvement of external counseling resources. Maintain updated, comprehensive care plans reflecting current problems, goals, and interventions. Complete initial and ongoing psycho-social assessments, identifying concerns related to financial means, insurance, living arrangements, caregiver stressors, and community resource needs. Facilitate placement in long-term care as warranted and monitor adjustment. Support and facilitate advance planning including living will, POA documents, funeral arrangements, and body/organ donation.
Required Qualifications
- Master of Social Work (MSW) from a CSWE accredited program
- at least 1 year of healthcare experience
- state licensure as required and applicable for the state in which you practice
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Articulates and embraces hospice philosophy
- Regular standing, walking, and manual dexterity
- ability to lift and move objects up to 50 pounds
- Visual acuity including close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Routine use of standard medical equipment and tools associated with clinical care
Desired Qualifications
- Bachelor's degree in social work or related field
- 1 year of healthcare experience (for locations where an MSW is available to supervise)
