Job Summary
Assess psychosocial status of terminally ill patients and families, providing anticipatory grief counseling, emotional support, and end-of-life education. Monitor family dynamics, caregiver ability, and safety risks to develop interventions and participate in individualized care plans. Assist with funeral arrangements, financial and legal decision-making, and facilitate care conferences with the hospice team. Provide bereavement follow-up and coordinate nursing home placements, respite stays, and travel contracts. Maintain clinical records and attend interdisciplinary group meetings. Requires a Master's in social work, Minnesota licensure, two years of healthcare experience, and a valid driver's license for local travel. Flexible weekday schedule from 8am-4:30pm with no weekends or holidays.
Required Qualifications
- Master's Degree in social work
- Licensed Graduate Social Worker in good standing in Minnesota
- Two or more years of social work experience in a health care setting
- Valid Drivers License
- proof of insurance
- satisfactory motor vehicle record
- Ability to communicate effectively in both written and verbal formats
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
