Job Summary
Provide coordinated in-home hospice social work services to patients of all ages, including psychosocial assessments, analysis, counseling, and referrals to address patient/family needs; report to Director of Nursing; works within an interdisciplinary team to support patient care planning and ensure compliance with agency philosophy and standards through ongoing education and resource coordination.
Required Qualifications
- Education: Bachelors or Master Degree in Social Work from a state accredited program
- Licensure: Texas Certification; Texas Drivers License
- Experience: One (1) year social worker experience in a healthcare agency.
