Job Summary
Conduct psychosocial assessments within five days of admission, identifying emotional, social, financial, and environmental resources to inform the patient's plan of care. Participate in IDG, QAPI, and discharge planning meetings while evaluating patient and family responses to medical social work interventions. Maintain written clinical records, including initial evaluations and daily notes, and coordinate weekly Case Manager visits. Act as a consultant to the health team regarding social and environmental factors, helping patients utilize community resources while assessing coping abilities with the patient's dying process. Offer continued support to IDG if services are declined. Requires a Master's in Social Work, licensure, and reliable driving. Pay is $40 per visit plus $65 per admission with mileage reimbursement.
Required Qualifications
- Master's in Social Work (LMSW)
- Must be a licensed driver with an insured automobile in good working order
- The ability to communicate well, both verbally and in writing
