Job Summary
Provide emotional, psychosocial, and practical support to hospice patients and families while assessing needs to develop appropriate care plans. Deliver counseling, crisis intervention, and grief support as required, and connect clients with community resources and services. Advocate for patient and family needs, dignity, and quality of life through collaboration with nurses, physicians, chaplains, and other team members. Maintain accurate, confidential documentation in compliance with HIPAA and hospice standards. This part-time role in Wichita, KS, offers a salary range of $45,988.26 to $55,383.71 with full benefits including health, dental, and vision insurance.
Required Qualifications
- A degree in Social Work or a related field
- Appropriate social work licensure or credentials
