Job Summary
Hospice Social Worker providing psychosocial services to terminally ill patients and families. Responsibilities include conducting initial psychosocial and bereavement risk assessments, facilitating referrals to community resources (including financial assistance), counseling patients and families, assisting with discharge planning and placement decisions, participating in interdisciplinary team meetings, documenting per agency guidelines, mentoring staff, and educating patients/families on disease progression and self-care techniques. Requires MSW, Ohio licensure, and at least 1 year healthcare experience with terminal illness; position available as a part-time role in Lebanon, Ohio.
Required Qualifications
- Master's degree in social work from a school of social work accredited by the Council on Social Work Education
- Active Ohio Social Work license
- Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements
