Hospice Social Worker

Job Summary
Hospice Social Worker provides psychosocial services to terminally ill patients and families. Conducts initial psychosocial and bereavement risk assessments within five days after Hospice election; facilitates referrals to community resources and financial assistance; assists with placement of patients in facilities for basic care or respite; offers psychosocial and anticipatory grief counseling; assesses psychosocial status and documents findings; participates in care planning and interdisciplinary team meetings; prepares social histories, evaluations, and interventions; ensures timely clinical documentation and discharge planning; mentors other personnel, supports resident/family education on disease progression and self-care techniques as ordered by physicians.
Required Qualifications
- Master's degree in social work from a school of social work accredited by the Council on Social Work Education
- Active Ohio Social Work license
- Valid driver’s license and reliable transportation, insured in accordance with Ohio requirements
- Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness; Hospice or home care experience preferred but not required
