Job Summary
Assess psychosocial status of patients and families regarding terminal illness, conduct social evaluations, and provide crisis intervention as needed. Implement standards of care by participating in interdisciplinary group meetings, developing individualized plans of care, and assisting with advanced directives and funeral arrangements. Educate teams on cultural needs, coordinate with physicians and community agencies, and maintain accurate clinical notes. Report suspected abuse, neglect, or abandonment to appropriate authorities within required timelines. Participate in on-call duties, perform mandatory reporting to the Department of Health, and fulfill case assignments while maintaining professional development through continuing education.
Required Qualifications
- A graduate of a Master's program in Social Work (MSW) from a school accredited by the Council on Social Work Education
- Minimum of one (1) year experience in health care
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
- Prolonged or considerable walking or standing
- Visual acuity and hearing to perform required social work skills
Desired Qualifications
- hospice experience preferred
- Understands hospice philosophy, and issues of death/dying
- Experience in hospice care preferred
- Demonstrates good verbal and written communication, and organization skills
