Job Summary
Conduct comprehensive bio-psychosocial assessments and ongoing reassessments for patients and families navigating end-of-life care. Provide emotional support, education, and counseling while advocating for clients and coordinating care with physicians, nurses, therapists, chaplains, and volunteers. Assist with advance directives, POLST forms, and guardianship, then connect families with community resources, transportation, respite care, and financial assistance programs including VA, SSI, Medicare, Medicaid, and private insurance. Participate in daily team huddles and interdisciplinary group meetings to set meaningful goals and ensure adherence to care plans. Full-time, 1.0 FTE role with a Master's degree in Social Work (MSW) and current Montana license required. Located in Helena, MT.
Required Qualifications
- Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
- Current Montana social work license (LSW or LCSW) or actively working toward licensure
- Minimum of one year of social work experience in a healthcare setting
Desired Qualifications
- Hospice, home health, palliative care, or community-based care experience
- Knowledge of chronic disease management, end-of-life care, and psychosocial support services
- Strong communication, counseling, advocacy, and care coordination skills
- Valid driver's license and ability to travel within the service area
