Job Summary
Hospice Social Worker (MSW) in a full-time, day-shift role providing psychosocial assessment and support across home, long-term care facilities, and hospital settings. Conducts clinical assessments of patients’ psychosocial, financial, environmental, and community needs; delivers crisis intervention and supportive counseling around end-of-life issues; participates in discharge planning and care coordination; collaborates with an interdisciplinary team to develop and implement Plan of Care while ensuring regulatory compliance and documentation. Requires Master of Social Work and applicable Washington social work licenses; preferred one year of post-master’s experience in a healthcare setting. The role emphasizes holistic, patient-centered care, family support, and coordination with external partners to optimize quality of life for patients and families. Compensation range: $35.81-$51.93 per hour.
