Job Summary
Assess psychosocial status of patients and families regarding terminal illness, conduct social evaluations including family dynamics and suicide risk, and counsel caregivers on stress and coping. Provide crisis intervention, educate the interdisciplinary group on cultural needs, and assist in preparing advanced directives. Maintain clinical records, facilitate funeral and legal planning, and serve as a liaison between patients and community agencies. Participate in quality assessment teams and regularly scheduled interdisciplinary meetings to support symptom management. Schedule Monday through Friday, 8am to 5pm, with coverage in Roseburg and surrounding areas. Requires a master's degree in social work and two years of supervised healthcare experience.
Required Qualifications
- Master's degree in social work from an accredited university
- Undergraduate B.A. Degree in Social Work
- Minimum of two (2) years of documented supervised experience in health care
- Hospice experience
- Understanding of hospice philosophy
- Understanding of issues of death/dying
- Flexibility in work hours
- Ability to travel throughout the assigned Bristol Hospice service area
- Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
