Job Summary
Assess psychosocial status of patients and families regarding terminal illness, providing crisis intervention and counseling on stress and coping. Carry out social evaluations to identify family dynamics, caregiver abilities, and risks for suicide or neglect, then plan and execute interventions. Educate the interdisciplinary group on cultural needs and assist in preparing advanced directives while participating in regular team meetings. Maintain accurate clinical notes, communicate condition changes to physicians, and serve as a liaison between patients and community agencies. Report suspected abuse, abandonment, or neglect to appropriate authorities immediately. Participate in on-call duties, ensure continuity of care, and support equipment and service arrangements.
Required Qualifications
- A graduate of a Master's program in Social Work (MSW) from a school accredited by the Council on Social Work Education
- Minimum of one (1) year experience in health care
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
- Prolonged or considerable walking or standing
- Visual acuity and hearing to perform required social work skills
Desired Qualifications

- Understands hospice philosophy, and issues of death/dying
- Experience in hospice care preferred
