Hospice Social Worker- MSW

Conduct timely psychosocial assessments of patients and families/caregivers, develop and support individualized care plans, and provide counseling on stress and coping challenges. Maintain accurate documentation, assist with applications for SSI, Medicaid, and hospice benefits, and coordinate community resources including social, health, and welfare services. Serve as a liaison between patients, families, caregivers, and community agencies while delivering exceptional service to referral sources and interdisciplinary team members. Requires current Ohio Social Work Licensure, minimum one year of healthcare or hospice experience, and a Bachelor's or Master's degree in Social Work.
