Job Summary
Deliver varied social work services to hospice patients and families, including initial emotional, spiritual, and psychosocial assessments, ongoing counseling, and bereavement support. Conduct goal-directed care plan development, crisis intervention, and community outreach while advocating for patient needs within the IDG team. Provide short-term crisis intervention and family counseling as required, documenting comprehensive assessments and maintaining contact with community agencies for referrals. Participate in IDG conferences and staff meetings, ensuring all visits and contacts are recorded per policy. Requires MSW degree, one year of hospice experience, and ability to work independently; Monday-Friday 8a-5p schedule with competitive benefits including tuition reimbursement and mileage allowance.
Required Qualifications
- MSW degree from a school of social work accredited by the Council on Social Work Education
- one year of experience in hospice care
- Minimum 2 years of experience as a social worker
- Active patient contact within past three years
- Must possess a valid driver's license

- Ability to work independently, make accurate, and at times, quick judgments
- Ability to respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural and religious modes
- Must be able to lift and/or move up to 25 pounds
