Job Summary
Conduct psychosocial, spiritual, and educational assessments for patients and families, then advocate for goals and coordinate transition planning with the interdisciplinary team. Round on patients to anticipate needs and collaborate to formulate, implement, and revise individualized Plans of Care within the electronic health record. Provide education and interventions regarding terminal illness and bereavement, while referring families to community resources and assisting with legal documents like Power of Attorney. Document all interventions and outcomes, report significant changes or incidents to the IDT, and participate in on-call rotations, death visits, and staff precepting.
Required Qualifications
- Bachelor's in Social Work (BSW)
- Current, unrestricted Social Work License in state of employment
- One-year paid medical social work experience
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Intermediate Microsoft Office skills
- Intermediate electronic health records skills
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
