Job Summary
Conduct initial psycho-social assessments of patients and families within five days of admission to contribute to the comprehensive care plan. Provide individual and family counseling to address mental and emotional status, identify religious or cultural needs, and evaluate financial or environmental concerns by directing families to community resources. Assist with advance directives, durable power of attorney, funeral arrangements, and crisis intervention support while acting as a liaison to the Interdisciplinary Group. Visit patients one-on-one in their home environments within the Southern Twin Cities Metro area to ensure safety and comfort.
Required Qualifications
- Graduate of a Master's program in Social Work accredited by the Council on Social Work Education
- Licensed to practice as a Social Worker in the state of Minnesota
- License is active and in good standing
- Minimum of one (1) year's experience in health care
- Able to travel within geographic area serviced by office
- Valid drivers license
- Reliable transportation
- Proof of auto insurance

- All offers of employment are contingent upon successful completion of drug screening
