Hospice Social Worker

Job Summary
Provide home-based hospice social services to patients/families addressing end-of-life issues, including care management, counseling, and advocacy. Work as part of a hospice team (RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT/OT/SLP) to implement the patient’s plan of care and goals; monitor psychosocial needs; conduct patient assessments; document progress toward goals; provide counseling and crisis intervention; consult with family/caregivers on the care plan and resource planning.
Required Qualifications
- Bachelor's degree in social work
- Licensed to practice as a Social Worker in the state in which employed (active and in good standing)
- Hospice experience preferred
