Hospice Social Worker - Gaston County

Job Summary
Provides psychosocial assessments of patients and families, develops and evaluates care plans addressing emotional, social, and environmental factors related to diagnosis and end-of-life care, and collaborates with an interdisciplinary team to deliver culturally sensitive hospice services. Roles include family meetings, education on hospice philosophy, resource referral, documentation per VIA guidelines, and ongoing coordination to facilitate transitions to grief care and supportive services. Requires licensure as an LCSWA/LCSW (NC) and LISW-CP (SC); MSW preferred for clinical social work practice; responsibilities include advocacy, referral coordination, and maintaining professional licensure.
Required Qualifications
- Bachelor’s degree required if working as a BSW
- Master’s degree in social work from a CSWE-accredited program if working as MSW
- Be licensed as a LCSWA or LCSW in NC and LISW-CP in SC
