Job Summary
Conduct in-person psychosocial assessments of patients and families within five days of admission per Medicare guidelines while providing emotional support, counseling, and crisis intervention. Self-organize, schedule, and triage visits based on clinical need to assist with advance care planning and end-of-life decision-making. Connect families with community resources and facilitate discussions regarding patient goals, family dynamics, and bereavement needs. Collaborate closely with nurses, physicians, chaplains, and aides to advocate for patient dignity and quality of life while completing timely documentation. Requires a Master's in Social Work from a CSWE-accredited program, Massachusetts licensure, and three years of recent experience in acute, long-term care, or hospice.
Required Qualifications
- Master's Degree in Social Work from a CSWE accredited program
- Licensed as a Social Worker through the Massachusetts Board of Social Worker
- Three years recent (within the past five years) experience in acute, long-term care or home care/hospice
- Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies
- Strong communication, counseling, and interpersonal skills
- Ability to work independently while remaining an engaged member of a multidisciplinary team
- Compassionate, empathetic, and patient-centered approach to care
