Job Summary
Coordinate patient and family needs through a holistic approach to achieve health care goals, utilizing an interdisciplinary treatment plan. Assist clients in coping with psychosocial challenges by consulting, educating, and facilitating referrals to other healthcare professionals and programs. Participate in team meetings and required in-service trainings while maintaining electronic documentation. This role covers everything south of Addison, IL, three days a week with flexible scheduling. Family Home Health offers a compassionate environment for experienced Medical Social Workers to support post-acute care services.
Required Qualifications
- Minimum of 1 year of Medical Social Work experience
- LCSW
- Compassionate and caring demeanor
- Ability to build a rapport with clients
- Excellent communication skills
- Customer service skills
- Well organized
- Comfortable with electronic documentation
- This position is 3 days a week
- Days can be flexible
- Primarily covers everything south of Addison, IL
Desired Qualifications
- HCHB EMR experience
- Prior Hospice care experience
