Hospice Social Worker

Job Summary
Conduct psychosocial assessments within five days of admission, evaluating emotional, social, financial, and environmental resources to identify interventions and goals for the patient's plan of care. Participate in IDG, QAPI, and discharge planning meetings while observing and recording changes in patient response to treatment. Maintain written clinical records, including initial evaluations and daily notes, and act as a consultant to the health team regarding social and environmental factors. Assess family coping abilities with the patient's dying and help patients utilize community resources. Confirm weekly scheduling of visits with Case Managers to facilitate care coordination.
Required Qualifications
- Licensed by GA State Board of Social Work Examiners
- Master's degree in social work from a graduate school of social work accredited by the Council on Social Work Education
- Licensed Clinical or Master Social Worker (LCSW or LMSW)
- At least one year of post-master's social work experience in a health care setting
- Ability to reliably handle and maintain confidential information
- Licensed driver with an insured automobile in good working order
- Ability to communicate well, both verbally and in writing
