Job Summary
Hospice Social Worker providing direct psychosocial services to terminally ill patients and families. Responsibilities include psychosocial assessments, bereavement risk assessment and intervention, monitoring family dynamics and caregiver safety, developing individualized plans of care with patient/family, coordinating funeral and legal/financial planning, facilitating care conferences, and coordinating placement and travel needs. Requires MSW, LSW in Minnesota, and two years of healthcare social work experience; valid driver’s license with ability to travel locally. Salary range $30.00-$35.00 per hour; full-time exempt position based in Saint Cloud, MN.
Required Qualifications
- Bachelor’s Degree in social work
- Licensed Social Worker in good standing in Minnesota (LSW)
- Two years of social work experience in a health care setting
- Valid Driver's License with satisfactory motor vehicle record
- Ability to communicate effectively in written and verbal formats
- Proficient with Microsoft Office Suite
- Ability to follow safety rules and policies (HIPAA, Vulnerable Adult and Abuse Policy, Right to know, etc.)
