Job Summary
Conduct psychosocial assessments within five days of admission, identifying emotional, social, financial, and environmental resources to inform the patient's plan of care. Participate in interdisciplinary group and quality assurance improvement meetings to develop and evaluate care plans while documenting changes in emotional and social factors. Confirm weekly scheduling of visits with case managers, assist the health team in understanding social determinants of health, and help patients utilize family and community resources. Provide ongoing support to interdisciplinary groups if patients decline direct social work services. Maintain written clinical records including initial evaluations, care plans, and daily notes.
Required Qualifications
- Bachelor's of Social Work
- At least one year of post-bachelor's social work experience in a health care setting
- Must be a licensed driver with an insured automobile in good working order
- The ability to communicate well, both verbally and in writing
Desired Qualifications
- Hospice experience preferred
